Centre Care is a new, state of the art, Rehabilitation and Wellness Facility, dedicated to our community and providing advancing medical care and services through a resident centered care approach for individuals in need of recovery after a hospital stay, sudden onset of illness, injury, effects of surgery or for those in need of care related to chronic disease management.

We are currently looking for dedicated, compassionate, and energetic LPN’s to join our Centre Care team.

LPN duties include:
Receiving and giving reports, providing thorough information regarding resident condition and needs.
Assuring that CNA’s are aware of any specific appointments or therapies of residents
Performing treatments correctly, following Doctor’s orders and Centre Crest Policy and Procedures.
Providing care for ill residents, and appropriate communication to supervisory staff.
Performing intermittent catheterizations, and as ordered insert Foley caths, operate suction machines;
Administering medication correctly following Centre Crest Policy and Procedure.
Complete correct charting of same.
Supervising and assisting with training of new CNA’s,
Assuring daily that residents are cared for correctly by all CNA’s
Serving meals to residents and assist or feed those residents who need to be fed
Assisting with admission of new residents.
Collecting data needed, including resident vital signs, etc
Performing correct, accurate, appropriate charting.
Remaining aware of specific documentation required by State and document accordingly
Collecting information to assist to prepare MDS, assist with Care Plan development
Managing time to allow for documentation needed, care tracker charting, documentation of meds and treatments
Transcription of physician orders per facility protocols.
Holding knowledge of State and Federal regulations and Centre Crest policy.
Having the ability to manage stress tolerance, pressure and anxiety

Requirements
Education: High School Diploma or its equivalent is preferred.
license to practice as a Licensed Practical Nurse issued by the Commonwealth of Pennsylvania
Experience: 1 year experience in a healthcare setting.
