Job description
Position responsible for both indirect and direct patient care using professional nursing skills and knowledge.
Analyzes health status of patients in determining nursing care needs, identifying goals and patient nursing care plans.
Responsible for ensuring quality care services are delivered to the patients.
RN has oversight of her caseload and all RN, LPNS that work with the patients on her caseload.
This is placed into territories.
Position requires second-tier problem solving and compliant resolution while maintaining high standards of ethics and integrity, including good communication, confidentiality, compliance with laws and company policies, and professional conduct.
RN Case Manager reports to the IDD, Home Health and Nursing Executive Supervisor
Primary Responsibilities:
Evaluates the effectiveness of the quality of nursing care provided.
