This position travels throughout Cameron and Elk counties.
Compensation for time and mileage occurs based on the reimbursement policy.

$500 Sign-on Bonus for Cameron and Elk counties
Yearly Salary $39,300-$40,500
This is a full-time position working 40 hours per week, 52 weeks per year.

Responsibilities
Provide case management services to adults and children with serious mental illness or emotional disturbance
Work with individuals and families to access, link, coordinate, and monitor needed services
Provide support and advocacy for clients and their families
Maintain client records and documentation
Collaborate with other professionals and agencies to ensure comprehensive care
Adapt work hours to the needs of the position, including occasional evenings, weekends, and on-call emergency services
Qualifications
Bachelor's Degree in a relevant field (e.
g.
, sociology, social work, psychology, gerontology, anthropology, political science, history, criminal justice, theology, nursing, counseling, or education) or a registered nurse; or a high school diploma and 12 semester credit hours in a relevant field plus 2 years of direct contact experience with mental health individuals
1-3 years of experience working with children and/or adults in a mental health setting preferred
1-3 years of case management experience preferred
Excellent verbal and written communication skills
Valid driver's license and access to a reliable automobile
Ability to work in various settings, including individuals' homes and community locations
Strong organizational skills and ability to remain composed in pressured situations
Must obtain the following clearances prior to hire: PA State Police Criminal History, PA Child Abuse History, FBI Criminal History, and Mandated Reporting Certificate
