Overview: The Palliative Care Nurse Practitioner is responsible for providing care and management of the patient with acute and chronic conditions in collaboration with a physician and other members of the healthcare team.
Provides clinical expertise to patients and their families, serves as a resource for health care providers, and participates in palliative care program development.
Work is performed with general independence and decision-making responsibility in accordance with palliative care clinical guidelines and scope of practice and the patient’s attending physician.
The Palliative Care Nurse Practitioner provides consultation to medical teams, patients and families, as well as nursing, social services, and spiritual care staff.
Participates in the assessment, planning, implementation and evaluation of palliative and primary care interventions.
(Primary care interventions are applicable to the outpatient setting.
) Responsible for providing palliative care clinical instruction to physicians, nurses, social workers, chaplains, and others as requested.
Participates in daily clinical rounds, providing patient specific education as it relates to palliative care.
Qualifications: Education: 1) Graduate of accredited Nurse Practitioner program.
2) Master’s degree in Nursing is preferred.
Experience: 1) Experience working as a Registered Nurse and/or Nurse Practitioner is preferred.
2) Palliative care, hospice care, or critical care experience preferred.
Knowledge, Skills, Abilities: 1) Current prescriptive authority approval.
(To prescribe and dispense drugs in Pennsylvania you must have a current Pennsylvania RN license and a current Pennsylvania CRNP certificate-see below).
2) Skill in applying and modifying principles, methods and techniques of medicine to provide on-going patient care.
3) Skill in gathering and analyzing patient data and interpreting findings.
4) Skill in preparing and maintaining records utilizing computer software.
5) Strong interpersonal and communication skills.
6) Ability to work productively and effectively within a complex environment, handle multiple/changing priorities and specialized equipment.
7) Critical thinking, analytical and problem solving abilities required as related to various aspects of patient care.
8) Knowledge of organizational policies, regulations, procedures, prescribed medical standards and common safety hazards and precautions.
License/Certification/Registration: 1) Licensed as a Registered Nurse in the State of Pennsylvania.
2) Certified Registered Nurse Practitioner in the State of Pennsylvania.
CRNPs that were certified by the Board after February 7, 2005, will be required to maintain current National certification in order to renew their Pennsylvania CRNP certification.
The new regulations require that the 30-hours of continuing education required for license renewal be taken within the CRNP’s specialty.
3) Meets the American Heart Association’s standards for Healthcare Provider Basic Life Support (BLS) and renews BLS every two years.
